Why Your Iron Is Low (Even Though Your Labs Say You're "Fine") — And What Your Hormones and Gut Have to Do With It

If you're a woman over 40 who's exhausted, brain fog, feeling cold, and your hair is coming out in the shower — but your last bloodwork came back "normal" — you're not imagining it, and you're not crazy. Feeling tired is one thing, you sleep it off, but feeling tired all the time, no matter how much sleep you get, and how well you eat, may be another thing that goes beyond low iron that your labs simply aren't built to catch.
This is one of the most common things I see in my coaching practice: women who are told their labs are fine, but who don't feel fine. And more often than not, when we dig into ferritin specifically — not just hemoglobin — the real story shows up.
Why "normal" doesn't always mean optimal
Most standard labs only flag iron deficiency once you're already anemic — meaning your hemoglobin itself has dropped. But you can run out of stored iron long before that happens. This is called iron deficiency without anemia, and it's real: iron isn't just for making red blood cells, it's essential for mitochondrial function — the energy-production machinery inside your cells, especially in your heart and muscles. When iron stores run low, those cells can't produce energy efficiently, even while your hemoglobin still looks perfectly normal on paper and you feel beyond tired.
The marker that actually tells this story is ferritin — your iron storage tank. And here's the gap: many labs consider anything above roughly 15–30 µg/L "normal." But most women don't start feeling like themselves again until ferritin is closer to 50–70 µg/L. That gray zone in between — technically "normal," functionally depleted — is where so many women get told nothing is wrong.
The hormone connection no one explains
Perimenopause doesn't just change your cycle — it changes your iron. Two things are happening at once, and together they create what researchers call a "double effect":
Heavier, less predictable periods. As estrogen and progesterone fluctuate in perimenopause, periods can become heavier and more unpredictable. Roughly 1 in 4 women over 41 experience heavy menstrual bleeding — and that blood loss adds up in iron terms faster than most women realize.
Declining estrogen makes it harder to absorb the iron you do get. Estrogen helps regulate a hormone called hepcidin, which controls how much iron your gut actually lets in — and it does this in a fairly direct way, by acting on the liver to keep hepcidin production turned down. As estrogen fluctuates and declines through perimenopause, that suppression lifts, hepcidin climbs back up, and your gut absorbs less iron from the exact same meals you've always eaten. So you're losing more and absorbing less, at the same time. (This is an active area of hormone research, and the picture is more nuanced than a single dial — but the pattern of lower estrogen tracking with higher hepcidin and reduced iron absorption shows up consistently in studies of women moving through perimenopause and menopause.)
Your gut is part of this story too
This is the piece that so often gets missed: you need adequate stomach acid to convert the iron in food into a form your body can actually absorb. Stomach acid production naturally declines with age, and it's also reduced by chronic stress, long-term use of antacids or acid-reducing medications, and gut conditions like atrophic gastritis. When stomach acid is low (a condition called hypochlorhydria), you can be eating all the right foods and still not be absorbing the iron in them. One simple tool I lean on heavily with clients here is lemon. A little acidity alongside meals supports the very digestive step that's often running low, which is exactly why my 5-Day Reset leans so heavily on lemon throughout the day — and it's made a genuinely dramatic difference in gut symptoms for so many of the women who present with less than optimal gut health. It's a small habit that touches both sides of this health equation: better digestion, and better conditions for absorbing the iron in your next meal.
This is why iron in isolation doesn't tell the full story. Hormones, gut health, and iron status are three threads of the same rope — pull on one, and the others move too.
Plant-based iron, done right: how to actually absorb it
Plant iron (called non-heme iron) is absorbed less efficiently than the iron in meat — often somewhere around 5–17%, compared to roughly 25% for iron from animal sources. But also worth noting is that too much heme iron can increase oxidative stress that damages the DNA of your colon cells, creating the set up for precancerous changes and eventually potentially cancer.
So if you're like me and choose to get your iron from plants, you still can get enough iron by making these intentional nutritional edits.
It means pairing and preparation matter more.
What boosts absorption:
Vitamin C, every time. Vitamin C converts iron into a more absorbable form and can meaningfully increase how much non-heme iron you absorb from a meal. Pair iron-rich plant foods with citrus, bell peppers, berries, or a squeeze of lemon — every time, not just occasionally.
Soaking, sprouting, and fermenting. Legumes and grains contain phytates, which bind to iron and block absorption. Soaking beans and lentils before cooking, sprouting grains, or using fermented options like sourdough measurably reduces phytate content.
Cooking your greens. Cooking spinach and other oxalate-rich greens (rather than eating them raw) reduces the compounds that compete with iron for absorption.
A cast iron pan. Cooking acidic foods (like a tomato-based sauce) in cast iron cookware adds meaningful elemental iron directly into your food.
Dark Chocolate 70%. A 1-ounce serving delivers about 3.4 mg of iron (19% DV) plus a solid dose of magnesium (~65 mg, 15% DV) — magnesium that supports energy production and muscle/nerve function, which matters since low iron and low magnesium often show up together as fatigue.
What blocks absorption — and needs distance from your iron-rich meals:
Coffee, black tea, and green tea — all contain tannins and polyphenols that bind to iron. Green tea gets a health halo, but it isn't the safe swap if iron absorption is your goal.
Dairy and calcium supplements
Ideally, give these about a 2-hour buffer from your iron-rich meals rather than pairing them together.
A note to my women clients who drink coffee first thing on an empty stomach — often to stretch a fasting window, or just to get moving in the morning, or to enjoy that coffee in your hands that cozy hug-in-a-mug feeling - I get it!: the fix isn't to force down food you don't want. It's to swap the drink.
Rooibos and hibiscus tea are both naturally low in the tannins that block iron absorption (rooibos even brings a bit of vitamin C to the cup), so they're genuinely gut- and iron-friendly. Plain hot water with lemon works beautifully too — and it does double duty, supporting the stomach-acid piece above at the same time. Or enjoy coffee after you eat and preferably 1-2hrs after to minimize interference with nutrient absorption.
A recipe that puts this into practice: Iron-Boosting Lentil & Citrus Bowl
This is exactly the kind of meal I mean when I say pairing matters — it's built specifically to maximize plant-iron absorption, not just contain iron.
Serves 2–3
Ingredients:
1 cup green or brown lentils, soaked 30 minutes to overnight if you have time, then rinsed
2 cups vegetable broth or water
2 cups kale or spinach, roughly chopped
1 red bell pepper, diced
1 small red onion, thinly sliced
1/4 cup pumpkin seeds, lightly toasted
1 orange, segmented
2 tablespoons fresh parsley, chopped
1 clove garlic, minced
Juice of 1 lemon
2 tablespoons extra virgin olive oil
Optional: 1 teaspoon blackstrap molasses (naturally iron-rich)
Sea salt and black pepper, to taste
Instructions:
Simmer the soaked, rinsed lentils in the broth for 20–25 minutes, until tender but not mushy. Drain any excess liquid.
While the lentils cook, massage the kale or spinach with a small squeeze of lemon juice and a pinch of salt — this softens the greens and helps reduce the oxalates that compete with iron absorption.
Whisk together the olive oil, lemon juice, minced garlic, molasses (if using), salt, and pepper for the dressing.
Toss the warm lentils with the massaged greens, bell pepper, red onion, and dressing.
Top with the orange segments, toasted pumpkin seeds, and fresh parsley.
Serve warm or at room temperature — and save your coffee or tea for an hour or two later, so it doesn't compete with the iron you just worked to absorb.

If food isn't enough: liquid vs. tablet supplements
Sometimes diet alone isn't enough to rebuild depleted iron stores, and that's a conversation to have with your provider, ideally with a ferritin level in hand rather than guessing. If supplementation is the right next step, here's what actually matters when choosing a form:

A few things that matter regardless of which form you choose:
Take it with vitamin C, not with coffee, tea, or a calcium supplement.
Consider alternate-day dosing. Research suggests taking the same total weekly dose every other day (rather than daily) can be just as effective while causing fewer GI side effects — likely because it works with your body's natural hepcidin rhythm rather than against it.
Morning tends to be best, when hepcidin is typically at its lowest and your body is most primed to absorb iron.
Don't guess long-term. Iron overload has its own risks, so supplementation is something to dose with actual ferritin numbers, not indefinitely on autopilot.
The bottom line
If you're over 40, exhausted, and you've been told your labs are "normal" — normal isn't the same as optimal, and it's worth asking your provider for a ferritin level specifically, not just a standard CBC.
Between shifting hormones, changing stomach acid, and iron absorption challenges, there's usually a real, physiological reason you don't feel like yourself — and real, practical levers to pull to feel back in control of your health.
